Enzymes break alcohol down into a substance called acetaldehyde, which is toxic, then break that down again into something harmless. It’s a good system. It just wasn’t designed for the volumes many people ask of it.

When drinking is heavy and repeated, three things happen in sequence. The liver’s normal fat-processing gets pushed aside while it deals with alcohol, so fat starts collecting inside liver cells.

Ongoing exposure to acetaldehyde and the inflammation it provokes then injures those cells directly. And every round of injury and repair leaves a little scar tissue behind, the same way skin does.

Can the Liver Recover After Quitting Alcohol? Why the Answer Depends on the Stage

Cirrhosis scarring is permanent, but stopping alcohol can halt further scarring and significantly improve long-term outcomes.

This is the section most people came here for, so it’s worth reading slowly and reading selectively.

There is a version of this answer that gets repeated everywhere, and it is misleading in both directions. “The liver heals itself” is true enough to give false comfort to someone with cirrhosis.

“Liver damage is permanent” is true enough to frighten someone with fatty liver into thinking their situation is fixed when it isn’t.

Neither is a fair description. Here’s the stage-by-stage picture instead.

How Much Alcohol Causes Cirrhosis? Understanding Your Risk

QUICK ANSWER

There is no single quantity that causes cirrhosis, and no threshold below which risk disappears. Research shows risk rising steadily with the amount and duration of drinking, with women developing liver damage at lower intakes than men (Roerecke et al., 2019).

Most people who drink heavily do not develop cirrhosis, but risk climbs the longer and heavier the drinking continues.

People want a number here a fair thing to want. The honest answer – it does not exist in the form the question assumes.

What research does show is a dose-response relationship. A systematic review and meta-analysis by Roerecke et al. (American Journal of Gastroenterology, 2019) pooling data across multiple studies found the risk of liver cirrhosis rising with increasing daily alcohol intake, with a steeper rise for women than for men at comparable amounts[6].

Duration matters alongside quantity. Years of moderate-heavy drinking can carry more risk than a short period of very heavy drinking.

So the shape of the answer is a slope, not a cliff edge. Every increment of drinking adds risk, and no amount of drinking guarantees the outcome.
